Medication dosages are particularly important in drug administration to children because of specific age and weight factors. It is estimated that one in every eighteen prescriptions that are written for children are the wrong medication or dosage. Because of weight and age requirements, overdose is more likely in children who are given the wrong medication than in adults.

  • Three babies recently died after being given an adult dosage of Heparin (10,000 units/mL), instead of the correct dosage (10 units/mL).  The bottles of both dosages had similar blue labels. A 12-year-old girl died after a nurse inadvertently gave her a double dose of a drug.
  • A 5-year-old autistic boy died because the wrong medication was administered during a controversial treatment for the disease. He received Disodium EDTA instead of Calcium Disodium EDTA. The generic names are Versinate and Endrate. They sound similar, both medications are clear, colorless, and odorless – hence the fatal mistake.
